Gastric emptying and secretion in patients with diabetes mellitus.

چکیده

The older workers claimed that the gastric secretion of acid was diminished in diabetes mellitus but more recent investigators have found conflicting results. Marks, Shuman, and Shay (1959), using Kay's augmented histamine test, found an acid secretory output in diabetics which differed little from the normal range. In contrast, Dotevall (1961a, b) found that acid output in diabetics was less than in normal subjects, and concluded that this lower secretory output was related to the higher blood glucose levels existing in diabetics (consistent with the depressant effect of glucose upon the secretion of acid by normal subjects and dogs, a result which has been claimed by a number of workers). This was not, however, confirmed by others. Glucose did not reproduce the marked suppressive effect of glucagon upon gastric secretion (Aylett, 1962), although blood glucose levels were comparable. The investigation of gastric empyting in diabetics is of some interest, since in peptic ulcer subjects Aylett (1962) has shown a relationship between the rate of gastric emptying and change in the blood glucose level. It would be interesting to discover, therefore, whether diabetics with their higher range of blood glucose level would show a slow gastric emptying rate compared with that of normal or of peptic ulcer subjects. Results are reported in a group of diabetic patients in whom gastric secretion and emptying rate were studied, using water test meals.

Delayed gastric emptying and gastric autoimmunity in type 1 diabetes.

OBJECTIVE Delayed gastric emptying and/or gastrointestinal symptoms occur in 30-50% of diabetic patients. Known contributing factors are autonomic neuropathy and acute hyperglycemia, but the role of gastric autoimmunity has never been investigated, although 15-20% of type 1 diabetic patients exhibit parietal cell antibodies (PCAs). We studied gastric motility in diabetes in relation to PCA stat...

Effects of a Protein Preload on Gastric Emptying, Glycemia, and Gut Hormones After a Carbohydrate Meal in Diet-Controlled Type 2 Diabetes

OBJECTIVE We evaluated whether a whey preload could slow gastric emptying, stimulate incretin hormones, and attenuate postprandial glycemia in type 2 diabetes. RESEARCH DESIGN AND METHODS Eight type 2 diabetic patients ingested 350 ml beef soup 30 min before a potato meal; 55 g whey was added to either the soup (whey preload) or potato (whey in meal) or no whey was given. RESULTS Gastric em...

Liraglutide in the treatment of type 2 diabetes mellitus: clinical utility and patient perspectives

Type 2 diabetes mellitus (T2DM) is a progressive disease associated with significant morbidity and mortality. There is good evidence that intensive glycemic control reduces the development and progression of complications in patients with diabetes. In order to achieve glycemic targets, patients often require a combination of oral therapy and/or insulin in addition to lifestyle modification. Unf...

Liraglutide in Adults with Type 2 Diabetes: Global Perspective on Safety, Efficacy and Patient Preference

Incretin-based therapies have been gaining much attention recently as a new class of therapeutics for type 2 diabetes worldwide. Among them, glucagon-like peptide-1 receptor agonist liraglutide has been rapidly increasing its global usage. Once daily injection of liraglutide significantly ameliorates glycemic control in patients with type 2 diabetes by enhancing insulin secretion and suppressin...

Prevalence of delayed gastric emptying in diabetic patients and relationship to dyspeptic symptoms: a prospective study in unselected diabetic patients.

OBJECTIVE Data on the prevalence of abnormal gastric emptying in diabetic patients are still lacking. The relation between gastric emptying and dyspeptic symptoms assessed during gastric emptying measurement has not yet been investigated. The aim was to investigate the prevalence of delayed gastric emptying in a large cohort of unselected diabetic patients and to investigate the relation betwee...
